Parents/Guardians will need to bring the following items with them:

• Proof of Residency, ex: electric bill, phone bill (no cell phone bills), lease agreement, rental agreement or a house settlement agreement in parent/guardian name.  All documents must be current within 30 days. (If you are living with someone and don’t have proof in your name, please contact the registration office about the required multi-residency form.)
• Photo ID of parent/guardian.
• Valid Shot Records (Georgia Immunization Certificate Form 3231).
  Health Department directions.
• Social Security Card (copy accepted but must be legible).
  Social Security Office directions.
• Student’s  Birth Certificate (copy accepted but must be legible).
• Proof of custody/guardianship if you are not the birth parent.

It is not necessary to bring your student with you to registration.  The PDC is open all year long for registration.

Students re-entering or transferring within Barrow County Schools must provide current proof of residency and photo ID.

Barrow County School students will not need to be registered if they are staying in the same Barrow County School zone.

Your student will not be registered unless all requirements are met.  There are no exceptions.

Please do not take these forms to the schools for registration.  The registration process must be done at the Professional Development Center located at 179 West Athens Street in Winder.

• Use black ink only to complete the forms
• Use 1 form per student that you are registering
• Parent or legal guardian only must complete and sign the Registration, Health and Medicine Policy forms.
• Complete the form in it's entirety, put N/A rather than leave blank.
• The Registration form will be validated once the registration process is completed for your student.
