Please complete this form if you would like to change your student's enrollment for the second nine weeks.

Please Note: The enrollment change will take effect on Tuesday, October 13th. If you request a change, your child's homeroom teacher will change. If you have any questions, please do not hesitate to contact your child's teacher or the school at 770-307-1540.
