If your rising 9th or 10th grade student desires to participate in our online Health and Personal Fitness course option this summer, complete this registration form at your earliest convenience. Please note, your registration will not be finalized until payment of $55 is received. You may contact Mrs. Jan Pollett at 770-867-7467 in order to make a payment by phone or in person.
