Clarification from the State of Georgia regarding the new Scoliosis #4400 form that was previously sent home.
*This must be returned to school for all 6th & 8th grade students within 90 days of the start of school.
*The Student will be part of the mass screening if only the demographic portion of the form is filled out.
*The Parent/Guardian may also choose to opt-out.
*The form must be completed by a physician or the health department.
*The School Nurse will not be screening individual students or completing the form.
