The 2022-2023 Georgia Student Health Survey, Georgia Parent Survey, and the Georgia School Personnel Survey will open Monday, November 7, 2022. These surveys provide schools with critically important data to help guide prevention and intervention strategies. The surveys will remain open through Friday, March 17, 2023.

If you DO NOT want your child to participate in the survey please print the letter linked below and return it to the school. All students will participate in the survey unless we have this form returned stating that you DO NOT want your child to participate.
